What is rhinoplasty?

Rhinoplasty — commonly called a nose job — is a surgical procedure that reshapes the nose to bring it into harmony with the rest of the face. It can refine the bridge, the tip, the nostrils, or the internal structures that affect breathing. Primary vs. revision rhinoplasty

A primary rhinoplasty is your first nasal procedure — typically planned to address aesthetic concerns, breathing function, or both at once. Revision rhinoplasty corrects the result of a previous nasal surgery, often after a less-than-ideal first outcome elsewhere. Revision cases are technically more complex and require specialized fellowship training, which Dr. Vranis completed under some of the country’s most accomplished facial surgeons.

How the procedure works

Rhinoplasty is performed under general anesthesia at our accredited surgical facility, typically taking two to three hours. Dr. Vranis uses a combination of open and closed techniques depending on what each individual nose requires. Modern technique focuses on preserving and reshaping the natural cartilage and bone of the nose, rather than removing or grafting more than necessary.

Most patients return home the same day, with a small splint protecting the nose for the first week. Detailed pre-op and post-op packets walk you through every stage.

What to expect during recovery

The first week of rhinoplasty recovery is the most noticeable: bruising and swelling around the eyes and nose are typical and resolve quickly with rest and head elevation. The splint is removed at one week, and most patients feel comfortable returning to desk-based work and social activities within seven to ten days.

Final results emerge gradually as residual swelling continues to settle over the following months. Subtle changes are still visible up to a full year after surgery — and the result is built to last.

The difference between open and closed rhinoplasty is the columellar incision. This allows the surgeon to directly visualize all the cartilage pieces of your nose from above. Closed rhinoplasty is great for small modifications and may also involve less tip swelling and a faster recovery overall. Dr. Vranis performs both open and closed rhinoplasty procedures customized to your needs.

Dr. Vranis pays particular attention to your ethnic features and does his absolute best to maintain those characteristics so that your nose matches the rest of your facial features.

The sutures and splints will stay in for a week. After that, patients may need to tape at nights for a month. They return to all activities including exercise and strenuous activity after 4 to 6 weeks.

Absolutely. Rhinoplasty, septoplasty and turbinoplasty is typically performed at the same time to improve the functional (breathing) and cosmetic aspects of the nose simultaneously.

Rhinoplasty involves manipulation of the nasal bones, the upper cartilages and lower cartilages that translate to the shape of the nose. A septoplasty (typically performed at the same time or in isolation for breathing only surgeries) involves manipulation of the septal cartilage. This is the cartilage that separates the right and left side of the nose. When it is severely deviated towards one side, it can obstruct airflow. Thus, removing a portion of it will allow air to pass more freely through that side of the nose.

Primary rhinoplasties will begin to look cute in a month, but final results will take 6 to 9 months as some of the residual swelling does take months to resolve. Revision rhinoplasties will take anywhere from 1 to 3 months to look cute, but residual swelling may take up to 18 months to completely resolve. Some revision rhinoplasty patients have very prolonged periods of swelling and may even require steroid shots to accelerate this healing process.

Patients that are unhappy with their prior rhinoplasty, may be good candidates for a revision. They must understand that this is typically a much more challenging operation with longer recovery times. However, well executed revision rhinoplasties is one of Dr. Vranis’s favorite types of surgeries.
