General Practice

Dr. Vranis is a Board-Certified, fellowship-trained plastic surgeon that combines experience and innovation to deliver exceptional results. Patients often compliment his compassion, empathy, humility and surgical excellence.

Patients in good health or have medical conditions that are well controlled who have a solid support system and have a good understanding of the risks and benefits of cosmetic surgery are excellent candidates for plastic surgery.

You will be initially greeted by our wonderful staff. You will meet with Dr. Vranis and discuss your aesthetic goals. He will do a thorough evaluation and make recommendations that will achieve your goals in the safest way possible. Then, you will meet with the patient coordinator who will review the cost of the procedure and all the logistics involved in surgery. The coordinator will work with you in the days/weeks after your consultation to guide you through the journey – from arranging pre-op & post-op appointments and aftercare facilities to making sure you have all the supplies and medications that way you can be stress-free and focus on recovery.

Patients will often schedule plastic surgery around upcoming life events (ie. weddings, birthdays, family trips, etc). Ideally, you will want 2-3 months of recovery to feel and look your best.

Absolutely, a virtual consultation is a great way to start your journey. You will meet Dr. Vranis and interact with the office to see if this is a good fit for you. Your surgical quote will be contingent on you coming to the office for an in-person pre-operative visit so Dr. Vranis can perform the necessary physical examination to solidify the surgical plan.

You will need to stay in Los Angeles for 3 to 7 days after surgery. Hotels such as the Beverly Wilshire, Peninsula, Beverly Hills Hotel are minutes away from the office and surgery center.

Facial Procedures

A facial rejuvenation surgery (NEOlift) involves lifting, re-volumization and skin improvement. This procedure was specifically designed by Dr. Vranis to address the three major components of facial aging. Fillers will add volume and will address one of the components of aging. Toxin procedure (ie. Botox, Daxxify, etc) will address dynamic skin wrinkles by weakening the underlying muscle that contributes to those movement lines.

This depends on a number of factors. Traditionally patients in their 50’s and 60’s were ideal candidates for facial rejuvenation procedures. As we have improved the safety and consistency of results, patients in their 40’s that have visible signs of deflation and descent – due to genetics or weight loss – are also excellent candidates for these procedures.

Skilled facial rejuvenation experts, like Dr. Vranis, are trained to perform all types of facelifts. Lifting of the SMAS – either through an extended deep place, SMAS plication, SMASectomy or High-SMAS techniques – is critical. Dr. Vranis has performed all of the techniques numerous times and thus, will customize the procedure to best suit your face and aesthetic needs.

Facelift and fat transfer for volume will last forever, however, the aging process will continue for all of us. Typically, patients will enjoy their results for 10 to 15 years. Some patients have genetics with higher-than-normal elasticity of their tissues which does contribute to faster than normal recurrence. To extend the longevity of a facelift result, one should do routine maintenance that includes, neurotoxin, lasers, microneedling and light therapies.

Eyelid surgery is a great way to refresh the peri-orbital area and look less tired and more refreshed. This can be done as a stand-alone procedure or in combination with a full facial rejuvenation procedure (NEOlift).

Rhinoplasty / Nose

The difference between open and closed rhinoplasty is the columellar incision. This allows the surgeon to directly visualize all the cartilage pieces of your nose from above. Closed rhinoplasty is great for small modifications and may also involve less tip swelling and a faster recovery overall. Dr. Vranis performs both open and closed rhinoplasty procedures customized to your needs.

Dr. Vranis pays particular attention to your ethnic features and does his absolute best to maintain those characteristics so that your nose matches the rest of your facial features.

The sutures and splints will stay in for a week. After that, patients may need to tape at nights for a month. They return to all activities including exercise and strenuous activity after 4 to 6 weeks.

Absolutely. Rhinoplasty, septoplasty and turbinoplasty is typically performed at the same time to improve the functional (breathing) and cosmetic aspects of the nose simultaneously.

Rhinoplasty involves manipulation of the nasal bones, the upper cartilages and lower cartilages that translate to the shape of the nose. A septoplasty (typically performed at the same time or in isolation for breathing only surgeries) involves manipulation of the septal cartilage. This is the cartilage that separates the right and left side of the nose. When it is severely deviated towards one side, it can obstruct airflow. Thus, removing a portion of it will allow air to pass more freely through that side of the nose.

Primary rhinoplasties will begin to look cute in a month, but final results will take 6 to 9 months as some of the residual swelling does take months to resolve. Revision rhinoplasties will take anywhere from 1 to 3 months to look cute, but residual swelling may take up to 18 months to completely resolve. Some revision rhinoplasty patients have very prolonged periods of swelling and may even require steroid shots to accelerate this healing process.

Patients that are unhappy with their prior rhinoplasty, may be good candidates for a revision. They must understand that this is typically a much more challenging operation with longer recovery times. However, well executed revision rhinoplasties is one of Dr. Vranis’s favorite types of surgeries.

Regenerative / Nanofat

Fat transfer is performed to augment certain areas of your face in a natural way. Nanofat is a liquid substance that does not have any volumetric component to it. Instead, it is full of regenerative cells and growth factors to improve the quality of skin.

Small volumes of fat transfer or nanofat can comfortably be done in the office under local anesthesia.

PRP/PRF is derived from your peripheral blood, while nanofat is derived from your adipose tissue. There is a significantly higher concentration of stem cells in adipose tissue compared to peripheral blood, thus, the regenerative potential of nanofat is more powerful than PRP/PRF.

Mommy Makeover

Most women will elect to combine a tummy tuck and a breast augmentation (with or without a lift). Extended tummy tucks, liposuction and fat transfer to the buttocks or breasts can also be considered depending on how extensive of a mommy makeover procedure you would like to have.

You should wait 6 months after finishing breast feeding to do a mommy makeover. This allows the hormones in your body to normalize and get back to your regular exercise plan.

Moms are basically real-life superheroes and can’t afford to take multiple extended periods of time to recover from surgery. Thus, we combine these procedures, in a safe way, so that they only need one surgery and recovery.

The tummy tuck portion of a mommy makeover will address the excess skin and fix the muscle separation simultaneously.

You will need help at home for the first week. You are restricted from lifting anything more than 5lb for the first week, 10lb for the second week and then back to most regular life activities after that.

A tummy tuck will remove the c-section scar, unless your c-section scar was placed unusually low.

Ideally, we recommend waiting until you are finished having children. But patients who have become pregnant after a mommy makeover do well with their pregnancy.

Cost & Financing

Insurance will only cover medically indicated procedures or portions of a procedure. We will work with your insurance company (Dr. Vranis is an Out of Network provider for all insurance companies and plans) if your surgery is related to breast cancer, trauma, breathing difficulties.

We work with financing companies (ie. Cherry) to make your surgery possible for you.

The surgical quote you receive includes the surgeon fee, facility fee, anesthesia fee, cost of implants and the first set of garments (if applicable).

The surgical quote will not include the cost of pre-op medical clearances, lab testing, mammograms, post op medications, after care facilities / pathology costs (if applicable) and basic medical supplies you should have at home. Your coordinator will review all the essentials and anticipated costs with you.

Safety & Recovery

The overwhelming majority of surgeries are performed at outpatient surgical facilities that hold the highest level of accreditation (QuadA Accreditation).

A board certified anesthesiologist is in the operating room at all times with you to ensure maximal safety throughout the duration of the procedure.

We will guide you through the healing process. Dr. Vranis employs meticulous surgical technique to close all incisions himself. This is by far the most important variable in scar healing. He also uses a specialized dressing that improves the appearance of scars. Lastly, we work closely with a dermatologist that specialized in scar appearance to further improve the appearance should you desire to meet with them.

Dr. Vranis will discuss with you when it is safe to return to work. This depends on your surgery and what type of work you perform (desk duties vs. active labor). Typically, patients are able to return to the gym for light exercises at 3 weeks and cleared for all exercises at 4 weeks.

Parental permission is required as is your assent as a minor. Procedures such as rhinoplasty can be performed in minors once they have reached skeletal maturity (around 15 years of age).

Very rarely do we need hospital level care for elective plastic surgery. This is only in the case for patients that have certain medical conditions that preclude them from having surgery in an outpatient facility in a safe manner.
